Check Eligibility Online
Visit beneficiary.nha.gov.in and verify your eligibility using mobile number, Aadhaar card, or ration card. This is a mandatory first step before applying for the card.
Locate Nearest CSC Center
Find your nearest Common Service Center (CSC) using the CSC locator on the official website or by searching 'CSC center near me'. Note down the address and contact number.
Gather Required Documents
Collect Aadhaar card (mandatory), ration card (if available), and ensure you have a working mobile number. Keep both original and photocopies of documents.
Visit CSC Center
Go to the CSC center with your documents. The CSC operator will verify your eligibility in the system and initiate the card generation process.
Complete Biometric Verification
Provide your fingerprint and photograph for biometric verification. This is mandatory for card generation and takes only 2-3 minutes.
Receive Your PMJAY Card
After successful verification, your card will be printed immediately. You'll receive a physical card with your card number, family details, and QR code. Keep it safe for hospital use.
Aadhaar Card (Mandatory)
Original Aadhaar card for biometric verification. Photocopy also required.
Ration Card (If Available)
Helps in faster verification. Not mandatory but recommended.
Mobile Number
Active mobile number for OTP verification and future communication.
Family Details
Names and Aadhaar numbers of family members to be included in the card.
Note: No income certificate, caste certificate, or other documents required. Only Aadhaar is mandatory.
After applying at CSC center, you can check your card status online:
- 1.Visit beneficiary.nha.gov.in
- 2.Select verification method (Mobile/Aadhaar/Ration Card)
- 3.Enter your details and verify OTP if using mobile
- 4.If card is generated, you'll see card number and download option
No Online Application: Complete online application is not available. You must visit CSC center or hospital for card generation.
Instant Card: Card is generated immediately at CSC center within 10-15 minutes after biometric verification.
Family Coverage: One card covers entire family (up to 5 members). All members get ₹5 lakh coverage.
Free Scheme: PMJAY is completely free. CSC centers may charge only ₹10-30 as service fee for printing.
Valid Nationwide: Card is valid in all states at any empanelled hospital across India.
❌ "Not eligible" message online
Solution: Your family is not in SECC 2011 database. Try different verification method or check state-specific health schemes. Call helpline 14555 for clarification.
❌ Biometric verification failed at CSC
Solution: Clean your fingers, try different finger, or visit during less busy hours. If issue persists, visit hospital Ayushman Mitra desk.
❌ CSC center charging high fee
Solution: Maximum service fee should be ₹30. If charged more, report to helpline 14555 or visit different CSC center or hospital.
❌ Aadhaar not linked to mobile number
Solution: Use Aadhaar number or ration card for verification instead of mobile number. Update mobile in Aadhaar later.
