What is Ayushman Bharat Card?

Ayushman Bharat Card, also known as PM-JAY Golden Card or Ayushman Golden Card, is a health insurance card issued under the Pradhan Mantri Jan Arogya Yojana (PM-JAY). This card provides ₹5 lakh annual health coverage per family for cashless treatment at over 27,000 empanelled hospitals across India.

The card is issued to eligible beneficiaries identified through the SECC 2011 database. It's a physical card with the beneficiary's photo, family details, unique Health Account Number, and a QR code for quick verification. The card is completely FREE and valid nationwide at all empanelled hospitals.

How to Get Ayushman Bharat Card

Check Your Eligibility

Visit beneficiary.nha.gov.in and verify if your family is eligible for Ayushman Bharat Card. Enter your mobile number, Aadhaar, or ration card details to check eligibility in SECC 2011 database.

Locate Card Generation Center

Find your nearest Common Service Center (CSC) or visit an empanelled hospital with Ayushman Mitra desk. CSC centers are available in most villages and towns across India.

Carry Required Documents

Take your original Aadhaar card (mandatory), ration card if available, and details of all family members. Ensure your Aadhaar has updated mobile number and address.

Complete Biometric Verification

At the center, provide your fingerprint and photograph for biometric authentication. This links your card to your Aadhaar and ensures secure access to benefits.

Receive Your Card

Your Ayushman Bharat Card will be printed immediately with your photo, family details, unique Health Account Number, and QR code. The card is valid for use at all empanelled hospitals nationwide.

Using your Ayushman Bharat Card for treatment is simple and completely cashless:

Go to any empanelled hospital. Find hospitals at hospitals.pmjay.gov.in

Show Card at Ayushman Mitra Desk

Present your Ayushman card at the Ayushman Mitra help desk in the hospital

Complete Biometric Verification

Provide fingerprint for identity verification linked to your Aadhaar

Get Cashless Treatment

Receive treatment without any payment. Hospital directly settles bills with NHA

Important: No payment required at hospital for covered treatments. If hospital asks for payment, report to helpline 14555 immediately.
