Check Your Eligibility Online
Visit the official PMJAY beneficiary portal at beneficiary.nha.gov.in. Select your state and search method (mobile number, Aadhaar, or ration card). Enter your details and verify with OTP. The system will show if your family is eligible for Ayushman Bharat card based on SECC 2011 database.
Choose Download Method
If already registered, you can download PDF directly from the portal. For first-time users, you have three options: (1) Visit CSC center with Aadhaar card, (2) Go to empanelled hospital Ayushman Mitra desk, or (3) Use Ayushman Setu mobile app for digital card access.
Complete Verification Process
For online download, verify your identity using OTP sent to registered mobile number. For CSC/hospital visit, provide Aadhaar card for biometric verification (fingerprint and photo). This links your card to your unique identity and ensures security.
Generate or Access Your Card
If visiting CSC/hospital for first time, the operator will generate your card after biometric verification. The card is created instantly with your photo, family details, and unique Ayushman Bharat Health Account Number. For existing cardholders, simply access your card details online.
Download PDF Copy
Once your card is generated, download the PDF copy from the beneficiary portal or Ayushman Setu app. Click the download button, and the PDF will be saved to your device. You can print this PDF or keep the digital copy on your phone for hospital visits.

❌ "Card not found" error during download

❌ OTP not received during download
Solution: Check if mobile number is registered with your card. Try resending OTP after 2 minutes. Ensure good network signal. If problem persists, try downloading using Aadhaar number instead.
❌ PDF download button not working
Solution: Clear browser cache and cookies, try different browser (Chrome/Firefox), disable ad blockers, ensure pop-ups are allowed, or try downloading from mobile app instead.
❌ Downloaded PDF is blank or corrupted
Solution: Update your PDF reader app, try downloading again with stable internet connection, use different device, or contact PMJAY helpline 14555 for technical support.
❌ Name or details incorrect on downloaded card
Solution: Visit nearest CSC center or hospital Ayushman Mitra desk with correct documents (Aadhaar, ration card). They will update your details in the system and regenerate corrected card.
