Visit Official Portal
Open beneficiary.nha.gov.in in your web browser. This is the official PMJAY beneficiary portal for checking eligibility.
Choose Verification Method
Select how you want to verify: (1) Mobile Number, (2) Aadhaar Card, or (3) Ration Card. Each method will show your eligibility status.
Enter Details
Enter your mobile number (for OTP method) or Aadhaar/Ration card number. Make sure to enter the complete number accurately.
Verify OTP (if using mobile)
If using mobile number, enter the 6-digit OTP received via SMS. This verifies your identity.
View Eligibility Result
Your eligibility status will be displayed. If eligible, you'll see your card details, family members, and card number. If not eligible, it will show 'Not Found'.
✅ Eligible & Card Generated
Your card details will be displayed including card number, family members, and download option. You can immediately download your card PDF.
⚠️ Eligible but Card Not Generated
You're eligible but need to visit CSC center for biometric verification and card generation. Carry your Aadhaar card.
❌ Not Eligible
Your family is not in the SECC 2011 database. Contact CSC center or call helpline 14555 for manual verification or grievance.

Download official PMJAY app from Play Store/App Store. Login with mobile number to check eligibility and card details.
📞 Helpline Number
Call 14555 (toll-free) and provide your Aadhaar/Ration card number. Operator will check and inform your eligibility status.
🏢 CSC Center Visit
Visit nearest Common Service Center with Aadhaar card. CSC operator will check eligibility and generate card if eligible.
🏥 Hospital Verification
Visit any empanelled hospital's Ayushman Mitra desk. They can verify your eligibility using their system.
❌ "Details not found"
Try: (1) Verify you entered correct number, (2) Try different verification method (Aadhaar/Ration card), (3) Check with CSC center
❌ OTP not received
Try: Check network signal, wait 2-3 minutes, request OTP again, or use Aadhaar/Ration card method instead
❌ Wrong mobile number registered
Solution: Visit CSC center with Aadhaar card to update mobile number in the system
